Immunizations
To help keep our school community healthy, all required immunizations should be completed before the first day of school. If immunization records are not on file, your child may be excluded from school until the requirements are met.
Students may attend school provisionally if documentation shows that at least one dose of the required immunizations has been given. However, if the remaining requirements are not completed within the provisional period, your child may be excluded from school until proper documentation is received.
Please provide a copy of your child’s official immunization record or a note from their healthcare provider for our records.

Students who are not fully immunized or who have an approved medical or religious exemption may be excluded from school if there is a confirmed case of a reportable disease during the school year, in accordance with state health guidelines.
