Authorization for Medications in School
- A new form is required for each school year and must be dated after July 1st of the school year.
- For this school year (2025/2026), it must be dated after July 1st, 2025.
Authorization for Medication in School
Private Physical Exam Form
- This can be completed up to 1 year before the school year it is required for.
- You can also submit an After Visit Summary (AVS) for the visit
Private Dental Exam Form
- This can be completed up to 1 year before the school year it is required for.
Vaccine Exemption Form
- This does not need to be completed yearly.
